EZ Forms: Restore system + make LMS ready.

A data store structure bug had disabled the EZ Forms system. The application has now been restored. The system has been modified to be LMS Ready. System should now work in cases where the underlying construction permit (for license to cover) or the licensed facility (for modification of license) was from LMS instead of CDBS.
